As health systems and provider groups continue to adopt value-based care (VBC), their financial risk options are becoming more complex. In this session, attendees will learn how Duly Health and Care (formerly DuPage Medical Group) evolved its VBC model to deliver lower-cost, higher-quality care. To achieve continued VBC success, open communication between physicians and their participation in clinical decision-making is critical. This session will highlight Duly’s VBC journey, an overview of the participation decision-making process, and degrees of provider risk-sharing tolerance. The speaker will share insights into data and tools used to help make care navigation easier for patients and their care teams. While this VBC model is specific to senior patients, Duly engages in VBC contracts across its entire patient population, with more than 300,000 patients in a risk-bearing arrangement.
